Maybe, but I don't see why that would break it. I just add another
submenu instead of a simple menu entry. Even NT4 should know how to deal
with that. And the submenu does appear as you reported, just the text
doesn't appear.

Not really ;)
Just good organization and keeping unrelated stuff in their own files.

As you know, the 1.0.8 works well. And what do you mean with "new Visual
Studio"? I switched to VS.NET2003 a long time ago.

Can't be. If the shell extension just quits, then you wouldn't see any
more menu's until you restart your computer. And most likely, the whole
explorer would exit too, not just the shell extension.
